4 AI receptionists in our list mention hipaa-compliant practices on their websites (keyword-matched). Lowest published entry prices: Phonely ($0/mo), Upfirst ($24.95/mo), Allo AI Receptionist ($32/mo). This is a price sort, not a quality ranking. ReceptionistRatings tracks 37 AI receptionists, 27 of which publish prices.

What to check for hipaa-compliant practices: does it book straight into the scheduling software you already use, can it transfer urgent calls to a person, and how much do you pay per minute or call once you go past the included amount?

Short answer: Many AI receptionists say "HIPAA" somewhere on their site. Far fewer say they will sign a Business Associate Agreement (BAA), which is what a covered practice needs before patient calls go through the tool. On their own pages, four do: Allo (the only one with a published BAA price, $50/mo per workspace), My AI Front Desk, Upfirst AI (custom plan) and Phonely (Enterprise).
Order rule: vendors whose own page says they sign a BAA come first. Among those, a BAA you can add to a self-serve plan at a listed price, with no sales call, ranks above a BAA by quote. Then published price. Affiliate status does not change the order; Allo is an affiliate partner. We have not test-called these tools.
Verdict
- Pick: Allo AI Receptionist
- Best for: small healthcare teams that want to add a BAA without a sales call
- Starting price (verified): Business seat $32/mo billed yearly ($45 monthly) + AI Receptionist $32/mo per agent billed yearly + signed HIPAA BAA $50/mo per workspace (Business and Ultra). Call allowance not published. Allo says its "Pricing is early and can still change." Because the call allowance isn't published, you can't estimate the full monthly cost before a trial.

Cost at 3-minute calls (standard plans, not BAA plans)
These are the cheapest standard plans; a BAA may need a different plan or add-on. - Upfirst AI: $24.95 at 30 calls, $69.95 at 100, $159.95 at 300. - Quo: $39 at 30, $68 at 100, $145.50 at 300, including a one-user $19 Starter phone plan. - echowin: $28 at 30, $70 at 100, $160 at 300, including a $10 number, before the $500 HIPAA add-on. - Nextiva XBert: $99 at 30 or 100, $297 at 300. - My AI Front Desk: $99 at 30 calls (200 minutes); overage at 25 credits ($0.25) per voice minute is on its pricing page but not yet in our data, so higher volumes are not modelled. - Allo: not computable; call allowance not published.

Allo
Allo is a business phone system with an AI Receptionist add-on. Its security page says it signs a BAA on Business and Ultra for $50 a month per workspace, the only BAA price published by any vendor here. Allo's medical-practices page covers booking, rescheduling and insurance questions. Weak spot: no published call allowance for the AI add-on.

My AI Front Desk
Its homepage says HIPAA compliant with signed BAAs. Its pricing page lists Business-in-a-Box at $99/mo ($79 billed yearly) with 200 voice minutes and a Partner/Enterprise custom plan, but does not say which one includes the BAA. Ask before you sign up.

Upfirst AI
Upfirst AI has the lowest published per-call prices on this list, but its BAA is on a custom plan with no published price. See Upfirst alternatives.

Phonely
Phonely's plan table shows "HIPAA (BAA Signature)" on Enterprise, described as for organizations taking 1,000+ calls a month or needing HIPAA compliance.

HIPAA mentioned, BAA not found
Nextiva XBert, Quo, echowin and Trillet mention HIPAA but we did not find BAA wording on the pages we checked. That doesn't mean they won't sign one. It means you should ask for it in writing. See Nextiva vs Quo.
Bottom line
"HIPAA" on a homepage is not a BAA. If you want to add a BAA to a self-serve plan without a sales call, Allo is the only one that offers that at a listed price (it says its pricing "is early and can still change"). My AI Front Desk, Upfirst AI and Phonely say they sign BAAs, so get a quote from each. Don't route patient calls through any AI receptionist or answering service until the BAA is signed.

What do people ask most?
These are the questions we get most, answered from published vendor data.
What makes an AI receptionist HIPAA compliant?
No vendor page makes you compliant. If the tool handles protected health information, you need a signed Business Associate Agreement (BAA) with the vendor. This page lists only what each vendor says on its own site.
Which AI receptionists sign a BAA?
On their own pages (checked 2026-10-22): Allo (Business and Ultra, published price), My AI Front Desk (plan not stated), Upfirst AI (custom plan) and Phonely (Enterprise).
Does Quo sign a BAA?
Quo's pricing page says "HIPAA ready". We have not verified a BAA from Quo, so ask Quo directly before using it for patient calls.
How much does a HIPAA-ready AI receptionist cost?
Only Allo publishes a BAA price. Upfirst AI and Phonely quote their BAA plans, and My AI Front Desk does not say which plan includes it. echowin lists a paid HIPAA add-on without BAA wording.
Is there a contract?
Allo says no setup fees and no contracts. My AI Front Desk says no contracts and cancel anytime. Upfirst AI custom and Phonely Enterprise plans are quoted; ask for terms.
