What to check for medical offices: does it book straight into the scheduling software you already use, can it transfer urgent calls to a person, and how much do you pay per minute or call once you go past the included amount?
Affiliate disclosure: we earn a commission if you sign up for Allo AI Receptionist, Marlie AI through the links marked “affiliate link” on this page. We don’t link to other vendors; affiliate status never changes picks or order. Full disclosure.
Short answer: For a medical office, the first filter is whether the vendor signs a Business Associate Agreement (BAA), not price. Four do on their own pages: Allo, My AI Front Desk, Upfirst AI (custom plan) and Phonely (Enterprise). Allo is the only one where you can add the BAA to a self-serve plan at a listed price ($50/mo per workspace) without a sales call. Allo doesn't publish how many calls its AI add-on covers, so you can't estimate the full monthly cost before a trial.
Order rule: vendors whose own page says they sign a BAA come first. Among those, a BAA you can add to a self-serve plan at a listed price, with no sales call, ranks above a BAA by quote. Then published price. Affiliate status does not change the order; Allo is an affiliate partner. We have not test-called these tools.
Verdict
- Pick: Allo AI Receptionist
- Best for: small practices that want to add a BAA without a sales call
- Starting price (verified): AI Receptionist add-on $32/mo per agent billed yearly, on a Business seat ($32/seat billed yearly or $45 monthly), plus $50/mo per workspace for the signed BAA (Business and Ultra). Allo does not publish a call allowance. Allo says its "Pricing is early and can still change."

How the shortlist compares
Price table is generated from tools.json. Estimated monthly cost at 3-minute calls, monthly list prices, cheapest standard plan:
- Allo: not computable (no published call allowance). Signs a BAA on Business and Ultra, $50/mo per workspace.
- My AI Front Desk: $99 at 30 calls (200 minutes); higher volumes not modelled (overage not in our data). Says it signs BAAs; plan not stated.
- Upfirst AI: $24.95 at 30 calls, $69.95 at 100, $159.95 at 300. BAA on custom plan, price not published.
- Phonely: $0 at 30 calls (Free), $150 at 100 (Professional). BAA on Enterprise, price not published.
- Nextiva XBert: $99 at 30 or 100 interactions, $297 at 300 ($0.99 per extra). No BAA statement found.
- Marlie AI: $49 at 30 calls, $66.50 at 100, $199 at 300. No HIPAA or BAA statement found.
Assumptions: every call 3 minutes; minute plans bill calls × 3; Nextiva interactions counted as one per call; tiers without a published overage are used only up to their allowance. Script: batch5/calc.py.

Allo
Allo's medical-practices page says its AI receptionist books new and returning patients, reschedules appointments, texts back missed callers and answers common questions such as hours and insurance accepted. Its security page says: "On Business and Ultra, Allo signs a Business Associate Agreement for $50 a month per workspace." Allo is a phone system first: you pay per seat, then add the AI Receptionist ($32/mo per agent billed yearly). The weak spot is cost forecasting: Allo does not publish how many calls the AI add-on covers.

My AI Front Desk
Its homepage says "HIPAA compliant. Handle sensitive conversations with signed BAAs, encryption in transit and at rest, and strict access controls." Business-in-a-Box is $99/mo ($79 billed yearly) for 200 voice minutes. The pricing page doesn't say which plan includes the BAA, so ask first.

Upfirst AI
Upfirst bills per call: Starter $24.95 for 30 calls, Premium $59.95 for 90, Pro $159.95 for 300, Scale $299 for 600, with $0.70-$1.50 per extra call. It says spam and calls under 15 seconds are not billed. Its pricing page lists "BAAs for compliance: Business Associate Agreements for HIPAA-regulated businesses" under its custom plan, so the published prices above are not the BAA price. Ask for the custom quote first. See Upfirst alternatives.

Phonely
Phonely's FAQ says it "supports HIPAA-compliant deployments and can sign a Business Associate Agreement (BAA) for Enterprise customers", and its plan table shows HIPAA (BAA signature) on Enterprise. The free plan (100 minutes) is useful for hearing the agent before you talk to sales, but no patient data should go through it without the BAA.

Nextiva XBert AI Employee
Nextiva's AI receptionist page lists "Medical offices, dental practices, urgent care, therapy centers" and says it can schedule patient appointments, answer insurance questions and "maintain HIPAA compliance". It is $99/mo for 100 interactions, then $0.99 each (tools.json, checked 2026-10-19). We found no BAA wording on the pages we checked, so get one in writing before using it for patient calls. See Nextiva alternatives or Nextiva vs Upfirst.

Also mentioned: Marlie AI
Marlie has a medical and wellness page that mentions appointment booking, patient intake and transfers to staff, and says its system is "designed with security in mind". It does not say HIPAA or BAA on that page, so we left it off the picks. It is the lowest estimate at 100 calls ($66.50) if your calls carry no patient health information.

What we left out
Quo says "HIPAA ready" on its pricing page, but we have no verified BAA for it. Abby Connect, Ruby, Newo.ai and echowin mention HIPAA in our data; we did not verify a BAA on their pages this round. See HIPAA-compliant AI receptionists.
Bottom line
If you want to add a BAA to a self-serve plan without a sales call, start with Allo, keeping in mind it says its pricing "is early and can still change". My AI Front Desk also says it signs BAAs; ask which plan. If per-call cost matters most, ask Upfirst AI for its custom BAA quote and compare. Never route patient calls through any AI receptionist or answering service before the BAA is signed.

What do people ask most?
These are the questions we get most, answered from published vendor data.
Which AI receptionists sign a BAA for medical offices?
On their own pages (checked 2026-10-22): Allo (added to its self-serve Business or Ultra plan at a listed price), My AI Front Desk (plan not stated), Upfirst AI (custom plan) and Phonely (Enterprise).
Is Nextiva XBert HIPAA compliant?
Nextiva's page says it helps "maintain HIPAA compliance" and its footer says "HIPAA Compliant". We found no BAA statement, so ask Nextiva for one in writing.
How much does an AI receptionist cost for a medical office?
See the price table: list prices come from each vendor's pricing page. A BAA may need a different plan or add-on than the cheapest plan shown.
Is there a contract?
Allo says it has no setup fees and no contracts. Upfirst AI, Phonely, Nextiva and Marlie AI do not state contract terms on the pages we checked; custom and Enterprise plans are quoted.
Can I cancel?
Nextiva offers a money-back guarantee (tools.json). We did not find cancellation terms for the others on the pages we checked; confirm before signing a BAA plan.
Have you tested these?
Not yet. Test-call results are coming soon. The order follows the stated rule: BAA signers first, then a self-serve BAA at a listed price, then published price.
